The Patient Centered Medical Home (PCMH) is a model of care that puts patients first and provides an avenue for doctors to be involved in whole-person care, meaning that you receive a spectrum of care ranging from routine to urgent care, support with mental health and advice and support for making healthy changes and decisions in your life. Patients work with a personal clinician as their first contact and receive comprehensive, con- tinuous medical care from a team within the health care system, rather than floating among several disconnected doctors who treat you for various current ailments. That PCMH team shares information and collaborates with each other to ensure you – as a whole – receive proper, high-quality care. The PCMH practices are recognized by the National Com- mittee for Quality Assurance (NCQA), a leader in health care accreditation that is dedicated to improving the quality of health care. South Carolina is lucky to have nearly 400 NCQA-accredited Patient Centered Medical Home practices throughout the state, and over 100 of them are in the tri-county area.
